Ever wonder how you can create a specialty that takes care of everyone. And I mean EVERYONE! From one day old to 120 years old. Then create a building to house them and then mandate access to their care 24 hours a day seven days a week and then demand that they diagnosis and treat and manage every possible medical, nonmedical and social condition known to man. Oh yeah, and get it right every time while communicating with pissed off doctors all day long? That's a lot of pressure. I have no idea how accurate ER doctors are in their diagnostic evaluations.
