A hospitalist reader sent me this picture of an uncommon presentaion of a common disease: severe bilateral venous stasis ulcers. Welcome to hospitalist medicine. Where severe cases like this is our normal bread and butter. This is one case where bilateral cellulitis may actually apply.
Many patients I take care of are incapable of maintaining basic levels of personal care for any number of reasons. They may be homeless. They may be addicted to illicit drugs, alcohol or other prescription medications. They may be experiencing severe depression or anxiety. They may have schizophrenia, bipolar, obsessive-compulsive disorder or any other of a number of psychiatric conditions that prevent them from maintaining adequate self cares.
These deficits may present as a lack of personal skin hygiene, washing and bathing skin and hair and maintaining clean cloths free of putrid smells. They may also fail to maintain a basic level of self awarness with regards to their medical condition. I suspect that's the case in this pair of legs below. This picture shows a severe case of bilateral stasis edema ulcers. This is what happens when poor skin hygiene meets chronic leg swelling and failure to seek medical attention. While time may heal many wounds, I have a sneaky suspicion that time is not going to heal all these wounds, as this medical someecard below so eloquently explains.

What the hell is that? Nasty psoriasis or nasty diabetes skin ulcers?

ReplyDeleteOr do we need to go to rat bites...spider bites...socks too tight....
the lesions are bilateral suggesting a sistemic process, there is hiperqueratosis, and descamation suggesting the process is cronical.
ReplyDeletepsoriasis can cause descamation, but you can find it mostly in joints where there is pressure, knees elbows, not on anckle.
Peripheral Vascular Disease, you got two ulcers at the same time, same goes for celulitis, sames goes for lymphadema, and those are common affections.
my bet is GANGRENOUS PYODERM is a rare affection caused or related to common disease Inflammatory Bowel Disease.
what did you use to treat him/her?
or a diabetic with PVD? well could be diabetic people start to realize about their diabetis problems later than 10 years usually.

ReplyDeleteI agree with happy, only this would be admitted as bilateral cellulitis and the patient would be on IV antibiotics complaining about his diet, his nicotine withdrawal and the TV channels.
ReplyDeleteMy dad has leg ulcers, not as bad as pictured. The pups were a refreshingchange to see.....It is sunshining and somewhat windy, but I thank God I have my dad........DO SOMETHING NICE FOR SOMEONE TODAY!

YES MAYBE THAT IS OLD, BUT HE IS MY ONLY LIVING FAMILY AND I LOVE HIM. SAY WHAT YOU HAVE TO, BUT NOT ALL PEOPLE ARE NASTY, RUDE, SMOKING JACKASSES LIKE YOU SEEM TO THINK.....SOME HAVE DIABETES AND ARE TRYING TO CONTROL EVERYTHING. MY DAD IS IN THE HOSPITAL AND DOING BETTER. MY MOM PASSED ALMOST 2 YEARS AGO, AFTER A 59 YEAR MARRIAGE TO MY DAD. IT WOULD BE HARD ON HER TO SEE HIS LEGS, IT IS HARD ON ME, HOWEVER, IF I WERE IN A SIMIALR SITUATION, HE WOULD DO ALL IN HIS POWER TO HELP ME........SO TAKE THAT! RUDENESS IS NOT A PRETTY SIGHT, THIS TOO CAN LEAD TO POOR ATTITUDES AND POOR HEALTH.....BTW, WHEN MY MOM WAS DYING, THEIR HOUSE WAS BRONKEN INTO AND RANSACKED.........LIFE DOES GO ON AND GOD HAS PLANS, GOOD PLANS FOR ALL.......LET ME BE, AND I LET U BE.......GOD BLESS YOU
