My Patient, Mr. Tough Guy:
After discharging Mr. Guy for "kidney stone pain", which consisted of self induced bleeding of the finger and dripping blood into the urinalysis (blood in the urine can often indicated a kidney stone), and then refusing to prescribe narcotics for the fake pain, I dictated the following addendum to my discharge summary:
Addendum:
I learned upon discharge that the patient threatened to assault me physically. The patient indicated to nursing staff that his father is a Hell's Angel. He wished his father was present so he could "kick my ass". The patient indicated I am the kind of person that he knocked out in high school.
Dr PCP, I will not accept this patient ever, for any reason, ever, due to his threats to my life.
This makes it all the more interesting that months later I received a letter personally addressed to Dr Happy from Mr.Tough Guy apologizing for his abuse. He said it was the drugs talking. He says he's getting help. He's going to Narcotics Anonymous and straightening out his life.
Mr. Tough Guy, I commend you. It's better not to cave than to appease.
