Remember that INR is logrithmic and not linear so an INR of 28 is not that different from an INR of 14 - the number just seems scarier. However, in a 100 year old neither an INR of 14 nor 28 is a good thing.
ohio, As far as I'm concerned, anything above 4 is a moot point to me. Then it's a matter of:
Are they bleeding or are they not bleeding? If they are having clinically significant bleeding it doesn't matter if their INR is 2 or 20, it's too high.
I generally don't treat any INR below 6-8 unless they are actively bleeding. Above 8-10, they'll get a whiff of Vit K.
You can find a listing of my expenses as I travel down that fertility road. I know you're curious. Go on over and check it out in my new blog, Explanation Of Benefits. And if you think it's worth a minute of your time, contribute what you can. Who knows what could happen if all the back door games were thrown out the window and everyone knew exactly what everything cost. Take a gamble and contribute. It may just help save someone's life.
I Give This Blog
The Happy Hospitalist
Hospitalist medicine, my way. I am a board certified internist who only works in the hospital. No clinic, otherwise my name wouldn't mean anything. It would just be The Internist. And that sounds like a really boring television series. So instead, you can call me Happy Hospitalist. I give you permission to rip on me anytime you like. At some point or another, I'll probably be doing the same to you. Since this blog takes up a bunch of time, I risk my lovely wife cutting my internet connection at anytime with the garden clippers. If one day I stop posting, don't worry, I've been banned in favor of quality time and snuggle sessions. So I guess I win either way.
Some words of advice. Don't listen to me. I ain't your doctor. I ain't treating you. Don't take anything you read here as medical advice. It ain't. No way. No how. I may or may not be a doctor. I may be ahomeless lad with a stolen lap top and free internet. Any clinical cases you read here may or may not really exist. I ain't tellin'. I may just be a bad story teller. If any story sounds like you, it ain't you. Just accept that there are others that sound like you. If you have any questions I prefer you don't find me. Also, in case you have a problem with anything I write, all opinions are my own and not the opinion of any of my coworkers or people that I work with. That should go without saying.

Holy Crap!!!! An INR of 28! Was she popping them like M&Ms? Man she sure screwed up her kidneys. Is the renal insuffiency reversible?
So, INR is 28, age is 100, and BUN is 160- is that right?
yes.
That is correct
Remember that INR is logrithmic and not linear so an INR of 28 is not that different from an INR of 14 - the number just seems scarier. However, in a 100 year old neither an INR of 14 nor 28 is a good thing.
